Directions for requesting an Advisor
if you are interested in a
Major or Minor Degree in Botany

or use the

Hard Copy Method:

1.  RIGHT Click on one of the Links below;
     then select  "Save Link As" to upload the file;
     for filling in and printing, using your favorite OS and word processor.

2.  Once you've filled it out;
     Save your completed modified form on your computer for back-up purposes.

THEN
   
3A.  Print it, and turn it in to the Chief Departmental Advisor;

OR;

3B.  Send it as an attachment with an email to the Chief Departmental Advisor.
